Our Signature Priorities: How We Help

For the Plaza Health Network

The Foundation provides funds that allows for the development of patient-focused programs and services above and beyond that which is reimbursed by Medicare and Medicaid.

Programs Include:

Music & Memory is an innovative program that uses personalized playlists of music to help patients with Alzheimer’s disease and dementia.

The program is grounded in extensive neuroscience research about how our brains respond to music. While Alzheimer’s disease damages the ability to recall facts and details, it does not destroy the lasting connections between a favorite song and special music memories from important events.

Providing the Music & Memory program to patients and residents with dementia and other memory conditions improves awareness, mobility, socialization, temperament and overall quality of life.

The Virtual Family Reunion Program

Coming in 2017

The Virtual Family Reunion program features a high tech, state-of-the-art communications system that helps patients stay in touch with family and friends anytime, anywhere.

With the help of each center’s social work team, patients and residents will be able to participate in video calls with family and friends anywhere in the world from the comfort of their rooms.

The goal of the program is to reduce isolation and loneliness of residents whose family members may live in other cities or countries, or who may not be able to visit as often as they would like. The program will allow families to come together to celebrate a resident’s birthday, enable residents to virtually attend a family event or holiday party, or simply provide the opportunity to visit with a family member or friend. Ultimately, our objective is to help our residents stay connected, thereby enhancing their quality of life.

For short term patients recovering from surgery, an accident or illness, the objective is to help them stay in touch with family and friends in order to reduce stress and elevate mood – two important factors in recovery and healing.

With your support, we can bring the Virtual Family Reunion program to each of our centers.

The Foundation participates in and presents a variety of educational and experiential programs aimed at promoting health and wellness for Boomers, seniors, and elders in the South Florida community.

H.E.L.P. Health Education Learning

The H.E.L.P. (Health Education Learning Program) program is a free resource for the community, providing 3 ways for Boomers, Seniors and their families to get reliable information regarding their health concerns, answers to their questions from health and wellness experts in the community and resources to help them find the services they need.
